The Upper Chesapeake Health Link Van will be at the Delle Grove Apartment building, back parking lot at 53 E. Bel Air Avenue, in Aberdeen, on the third Thursday of each month from 1-4 pm.
Who is Eligible?
The Van Serves Harford or Western Cecil County Residents who are 19 years of age or older, have no current primary care provider, have no insurance except Medicare or straight Medical Assistance or Priority Partners MCO. *No Primary Adult Care (PAC); meet income requirements of:
Family of one: income less than $32,490
Family of four: income less than $66,150
What Kind if Care Would I Receive?
Medical management of health problems such as high blood pressure, diabetes and cholesterol; on-emergency sickness; preventative care and vaccinations; assistance with limited specialty care referrals; assistance in obtaining low-cost medication.
What Must I Do to Join?
First visit is free then sliding scale fee after that. Complete application and include proof of income – last 3 pay stubs, W2, disability/benefits letter, or alimony agreement/decree.
The Health Link Vans visits to Aberdeen are funded through a grant from the Dresher Foundation.
